About the Role
Our Adult Day Health program is seeking a dedicated LPN to join our team and provide direct nursing care, medication administration, and participant support in a social, community-based environment. This is a stable daytime nursing role focused on preventive care, wellness, and participant engagement.
- Administer medications and treatments per RN direction
- Record vitals and participant observations
- Assist with wound care and chronic disease management
- Maintain accurate, timely nursing documentation
- Communicate participant status changes to RN
- Promote participant safety and well-being
- Safety & fall prevention
This position meets all Massachusetts EOHHS ADH staffing and clinical standards (130 CMR 404), including CORI checks, TB/medical screening, and completion of mandatory training.
Requirements
Current Massachusetts LPN license
CPR/First Aid certifications
Experience with adult or senior populations preferred
Strong communication and organizational skills
