About the Role
Our Adult Day Health program is seeking a dedicated Social Worker to join our interdisciplinary team and help enhance the emotional, social, and functional well-being of our participants and their families. This is a daytime social worker role focused on social support, family services, and participant engagement.
- Conduct initial and ongoing psychosocial assessments of participants.
- Develop, implement, and update care plans in coordination with the interdisciplinary team (RN, Activity - Director, Program Director, and other clinicians).
- Provide supportive counseling, crisis intervention, and referrals to community resources.
- Lead or assist with family meetings, care conferences, and participant advocacy.
- Document all services in accordance with Massachusetts Adult Day Health (ADH) regulations and program policies.
- Maintain accurate, timely, and compliant participant records.
- Support participants with cognitive decline, behavioral challenges, or emotional distress.
- Collaborate with state agencies, hospitals, home care agencies, and community service providers.
- Participate in quality improvement, compliance audits, and regulatory reviews.
Requirements
Bachelor's or Master's degree in Social Work (BSW/MSW) from an accredited program.
Experience working with elderly adults, adults with disabilities, or medically complex populations.
Strong knowledge of Massachusetts elder services, community programs, and support systems.
Excellent interpersonal, counseling, and documentation skills.
Ability to work effectively in an interdisciplinary healthcare team.
CORI background check and other compliance requirements per MA regulations.
Licensed Social Worker in MA (LSW, LCSW, LICSW).
Experience in Adult Day Health, long-term care, home care, or community health.
